  • Diabetes

    Diabetes is a chronic health condition that is characterised by higher than usual blood sugar levels. Sugar from the food we eat converts into energy in our cells, with the assistance of a hormone called insulin. In people with diabetes, their body is not producing insulin (Type 1), not producing enough insulin or their insulin is not working efficiently (Type 2). This results in sugar remaining in the bloodstream, rather than being converted into energy in our cells. The result is higher blood sugar levels than considered normal. High blood sugar levels can result in further damage to our bodies. This is why it is important to improve your levels through diet and lifestyle changes.

     

    The different types of diabetes

    Type-1

    Type 1 Diabetes is when the body is no longer able to make insulin. As a result of this, the body is unable to turn glucose (sugar) into energy. Instead, the body uses fat to produce energy causing high levels of a dangerous substances to accumulate in their blood, leading to ketoacidosis, which is a potentially life threatening condition. For people with Type 1, the use of insulin can help to prevent this dangerous chemical build up.

    Type-2

    Type 2 Diabetes is the most common form of diabetes and affects about 85-90% of all people with diabetes. It is more common in older individuals, but can occur in anyone. In people with Type 2, their body’s ability to produce insulin is impaired or their body does not use the insulin being produced effectively. Risk is increased in individuals who are overweight or obese, have high blood pressure, poor diet and low physical activity levels.

    Under the heading of ‘pre-diabetes’ are two conditions:

    • Impaired Fasting Glucose – when blood glucose levels are higher than normal after an eight hour period of fasting.
    • Impaired Glucose Tolerance – following an Oral Glucose Tolerance Test, the two-hour blood glucose reading is higher than normal, but not high enough to classify as either Type 1 or Type 2.

    People diagnosed with pre-diabetes can prevent this progressing to the full disease by correct diet and lifestyle management.

    LADA – Late onset diabetes

    Latent autoimmune diabetes in adults (LADA), also known as ‘late onset’ or ‘Type 1.5’, is a form of Type 1 Diabetes that occurs in adults with a delayed onset. LADA is often initially diagnosed as Type 2 but there is no strong family history or other risk factors such as overweight, obesity, high blood pressure or inactive lifestyle. Treatment is initially with oral medications, which is not sufficient long term and inevitably insulin will need to be administered.

    Gestational diabetes

    Gestational diabetes occurs when higher than normal blood glucose levels appear for the first time during pregnancy. This most commonly occurs around the 24th to 28th week of pregnancy. Following the birth of the baby, the mother’s blood glucose levels usually return to normal, but there is an increased risk that Type 2 will develop later in life. The administration of insulin is common in women with gestational diabetes.

  • Liver and Kidney Disease

    The primary role of the liver is to eliminate toxins from the body, aid in the digestion of nutrients, particularly those traveling from our stomach before they rejoin our blood stream and produce blood protein and blood clotting agents.

    Many dietary and lifestyle factors can lead to the damage and inflammation of our liver, most commonly those caused by our affluent lifestyles:

    Dietary risk factors include:

    • Overweight or obesity.
    • A waist circumference of more than 80cm for women and 94cm for men.
    • Existing deranged liver enzymes or cholesterol levels (triglycerides, total cholesterol, elevated liver function tests).
    • Existing pre-diabetes or diabetes.
    • Alcoholism.

    Forms of liver damage can exist commonly as ‘fatty liver’, which is commonly caused by obesity and central weight gain, the prolonged use of regular drug therapy (e.g. steroids, opioids, chemotherapy) and, of course, alcohol.

    Often alcohol is responsible for other conditions such as Alcohol Steatohepatitis and cirrhosis, but it can exist also as non-Alcohol Steatohepatitis (NASH).

    Cirrhosis causes death of the liver cells, which is almost irreversible, and in the long term can lead to liver failure, massive nutrient imbalances, loss of appetite and severe malnutrition and weight loss.

    What is kidney nutrition?

    Looking after the health and well-being of our kidneys (renal system) is important at any age. The kidneys act as a filter, working to excrete waste products from our bodies through urine. These waste products are largely related to the food we eat.

    Many factors can affect your kidney function. These include age, poorly controlled Type 1 and 2 diabetes, cardiac problems, elevated blood pressure, being overweight or obese, episodes where you are very unwell (acute medical problems) as well as dehydration. When your kidneys are under more stress, it can mean that you have a greater chance of long lasting damage to your kidneys.

    What is malnutrition?

    Malnutrition is when a person is not getting enough food or the right kind of food, to meet their nutritional requirements. If the food an individual is consuming does not contain adequate amounts of the proper micronutrients, they can become malnourished and unwell, regardless of their total body weight.

    Preventing unintentional weight loss

    Some people find that due to various diseases or metabolic reasons, they are unable to maintain their body weight or experience unintentional weight loss. Situations where this is commonly seen is in cancer patients who are undergoing harsh treatments, or in elderly patients who have lost their appetite and struggling with food intake.

    When individuals are underweight or malnourished, they need to increase body weight to improve their health and prevent the increased risk of morbidity and mortality.

    Medical nutrition supplements for weight gain

    What is pregnancy nutrition?

    Nutrition during pregnancy can be divided into three categories:

    1. Pre-natal nutrition – the nutrition of the mother (and father) prior to conception. It’s very important to ensure that everyone is in the best physical condition to assist with fertility and conception, as well as the prevention of neural tube defects.
    2. Antenatal nutrition – the nutrition of the mother and baby during pregnancy. This time is vital for ensuring that the mother is consuming the correct nutrients to allow the baby to develop well and for the mother to stay strong and healthy.
    3. Post-natal nutrition– the nutrition of the mother and baby after birth. The mother’s diet is crucial following delivery of the baby, especially if she is able to breastfeed. In addition, returning to pre-pregnancy weight in a timely manner is necessary for long-term health.

    During pregnancy, there is a recommended range of weight gain based on your BMI. The higher your BMI is, the less weight you will be recommended to gain. It is important to keep your weight gain to the recommended level for the health of your unborn baby. This will also to help minimse the risk of gestational diabetes and to allow you to return to your pre-pregnancy weight in a timely manner.

  • Inflammatory Bowel

    What is inflammatory bowel disease?

    Inflammatory bowel disease refers to two chronic inflammatory disease of the bowel: Ulcerative Colitis and Chron’s disease.

    Chron’s disease is the more severe of the two. It involves inflammation affecting any area of the digestive tract from mouth to anus, causing inflammation of the entire bowel wall.

    Ulcerative Colitis is only found to cause inflammation of the large bowel and rectum. Inflammation only occurs in one layer of the bowel lining.

    The symptoms for both Ulcerative Colitis and Chron’s disease can include:

    • abdominal pain.
    • blood in the stool.
    • diarrhoea.
    • nausea.
    • loss of appetite.
    • fever.
    • unintended weight loss.
    • malnutrition.

    What is an eating disorder?

    Eating disorders describe a range of problems associated with food and body image. Unfortunately, in our society it is common for individuals to feel unhappy about their bodies and to be dieting or worrying about food. An eating disorder can result when these problems are extreme or interfere with an individual’s normal activities and quality of life. These concerns are considered to be psychological disorders.

    Eating disorders, if left untreated, can become a real psychological, medical and nutritional concern. Disordered eating behaviours can result in unhealthful eating styles, which lack essential nutrients for health, growth and development. This is particularly concerning in younger children and adolescents. Further, eating disorders can cause severe heart abnormalities, malnutrition, dangerously low body weight and even death.

    Anorexia Nervosa

    Anorexia Nervosa is an eating disorder characterised by severe food restriction, inappropriate eating habits or rituals, an irrational fear of weight gain, obsession with being thin as well as a distorted perception of one’s self. Anorexia Nervosa is more common in females than males and usually appears initially in adolescence.

    Binge Eating Disorder

    Binge Eating Disorder (BED) is a mental illness characterised by binge eating large amounts of foods in a short period of time. It can go unrecognised for a long time as most individuals have a body weight close to the healthy range or that fluctuates greatly.

    After a binge, you may try to diet or eat normal meals, leading to more bingeing episodes, creating a vicious cycle. Individuals with BED often have a distorted perception of ‘normal’.

    Binges can be followed by emotional and behavioural compensations such as:

    • Vomiting of food.
    • Misuse of diet pills, laxatives, and drug use e.g. methamphetamines, insulin (diabetics).
    • Preoccupation with body weight or shape and body image.
    • May commonly occur in those with a prior history of Anorexia Nervosa.

     Some common presentations may include:

    • Depression and anxiety.
    • Eating unusually large amounts of food, even when you’re not hungry.
    • Eating rapidly during binge episodes and until you’re uncomfortably full.
    • Frequently eating alone.
    • Feeling depressed, disgusted, ashamed, guilty or upset about your eating.
    • Feeling isolated and having difficulty talking about your feelings.
    • Frequently dieting, possibly without weight loss.
    • Losing and gaining weight repeatedly, or ‘yo-yo’ dieting.

    Bulimia Nervosa

    Bulimia Nervosa is an eating disorder as well as a severe mental illness. It can go unrecognised for a long time as most individuals have a body weight close to the healthy range. It is characterised by binge eating of large amounts of foods in a short period of time, followed by compensation such as:

    • Vomiting.
    • Fasting.
    • Excessive exercise.
    • Inappropriate use of medications to control weight.

Breastfeeding is the preferred source of nutrition up until about six months of age, but if you are unable to breastfeed, infant formula is also suitable.

  • Coeliac Disease

    What is Coeliac disease?

    Coeliac disease is an autoimmune condition where the body’s immune system reacts to gluten. Gluten is a protein found in wheat, rye, triticale and barley. This immune response causes damage to the small bowel lining, resulting in the malabsorption of nutrients. The symptoms associated with this disease vary greatly from person to person. The most common symptoms include bloating, wind, diarrhoea or constipation, iron deficiency, vitamin and mineral deficiencies, and fatigue.

    The only known treatment for Coeliac disease is a strict, long-term gluten-free diet.
